What is MyChart?

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:

    • View your health summary
    • View test results
    • View medications on file and request prescription renewals
    • View allerigies and immunizations on file
    • View medical history and current health issues
    • Access trusted health information resources
    • Download your health record
    • Communicate electronically and securely with your medical care team
    • Request medical appointments
    • View details about past appointments and hospital admissions

MyChart is available to patients who have been treated by a Nationwide Children's physician.

How do I sign up?

For security reasons, MyChart Activation Codes are only provided during your clinic appointment or Inpatient stay at Nationwide Children’s Hospital. Please request a MyChart Activation Code during your next visit. Our staff will give you a MyChart Activation letter with an activation code. Once you have your activation code, visit MyChart.Nationwidechildrens.org and click on the New User Sign Up Now button and follow the instructions.

What is an activation code?

An activation code is the unique identifier you will use, along with last 4 number of your Social Security number and your date of birth, to login to MyChart for the first time. An activation code will expire 60 days after it is generated.

How do I obtain access to my childrens’ medical records?

You will need to be authorized for proxy access for each child. Individuals may request proxy access to a minor’s MyChart account during a clinic visit. Proxy access will only be granted to parties with parental rights or legal guardianship over the patient and only to the extent that the party requesting proxy access can demonstrate the legal right to that patient’s medical information. Please notify your clinic if the proxy’s legal relationship with the account holder changes. Nationwide Children’s Hospital reserves the right to revoke proxy access at any time for any reason.

How do I choose my personal MyChart username and password?

During your first login to MyChart, click on the New User “Sign Up Now” button and follow the instructions. You will enter your activation code as well as create your unique MyChart username and password during this process. This is the username you will use every time you login into MyChart. Once created, your MyChart username cannot be changed. Choose a username and password that is unique to you and easy for you to remember. Avoid using a username or password that is easy for others to guess, such as your first or last name.

    • Your MyChart username must be between 6 and 30 characters long. You can use letters, numbers and the following special characters; period (.), underline (_), and ampersand (@).
    • Your password must be a minimum of 8 characters long and must include at least one number and one letter; special characters are allowed. Password cannot be the same as your username.

Why do I need an Internet email address to use MyChart?

When new information is sent out to your MyChart account you will also get an internet email to let you know that new information is available.

When can I see my test results in MyChart?

Most results will be released within 4 business days. This 4 day timeline also applies when you are activated on MyChart for the first time - meaning if you get activated today, you may not have any results released to you until 3 business days later. Some results have to be manually released to you by a clinician. If you have questions regarding your lab results, please contact the clinic directly.

If I send a message to my doctor's office, when can I expect a reply?

Every attempt will be made to respond to your Medical Advice message within 2 business days. Do not use MyChart for medical emergencies. If you feel you have a medical emergency, please seek care immediately or dial 911 as appropriate.

I cannot find my doctor/clinic in the drop-down list to send a message to, what do I do?

The list of recipients in the drop-down list is determined by many factors. The system will look at the appointments you have had in the past year as well as future appointment within 30 days. Not all clinics/doctors are available for you to send a message to. Please call the clinic directly if you do not have the option to send a message.

When will my appointment be scheduled after I submit an appointment request?

Every attempt will be made to accommodate your appointment request, however at times, the date, time, physician and/or location that you select may not be available. Allow up to 5 business days to receive a confirmation of your appointment details. You will either receive a message through MyChart or a phone call from a Central Scheduling Representative. Do not use MyChart for medical emergencies. If you fee you have a medical emergency, please seek care immediately or dial 911 as appropriate.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

How do I renew a prescription through MyChart?

You can go to "Message Center", click on "Get Medical Advice", then choose the appropriate provider/clinic and the subject of "Request Rx Refill". Be sure to include the medication name, your pharmacy name and phone number, and any other information that you feel is important. keep in mind a prescription renewal is needed only when you have no more refills remaining.

Log into MyChart and from the left menu, go to the Preferences section and select the appropriate option.

After Visit Summary

Patients can view visit summaries from hospital admissions, ED visits, and outpatient visits using the After Visit Summary within the Past Appointments feature within MyChart.

Content Linking

Content Linking allows patients to learn more about the clinical information that appears to them in MyChart. Topics enable for content linking appear as hyperlinks and are allergies, medications, test results, immunizations, and health issues. Either click the hyperlinked topic you want to learn more about (as in Current Health Issues) or click the “About This” hyperlink (as in Medications).

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Futher, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

What is the security question used for?

The security question and answer provides added security if you have forgotten your MyChart password or want to change it. You are required to answer your security question before being allowed to make a password change online.

I forgot my password. What should I do?

If you forget your password, or simply wish to change it, you can click on the “Forgot Password?” link on the MyChart Sign In page. You will need your MyChart username, Social Security number, and date of birth. You will then be asked your Password Reset question. If you answer this question correctly, you will be allowed to reset your password.

I forgot my username. What should I do?

If you forget your MyChart Username, you can recover your MyChart Username by accessing Forgot MyChart Username? link on the MyChart Sign In page. Your MyChart username is permanent and cannot be changed.

My activation code does not work. What should I do?

For your security, your activation code expires after 60 days and is no longer valid after the first time you use it. If the access has expired before you have completed the signup process, you will need to obtain another activation code from your clinic.

I was logged out of MyChart. What happened?

We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

Who do I contact if I have login issues or receive an error message?

Should you have technical issues, please email details of the issue, and include the full name, birthday and address for you and the patient whose chart you are having issues accessing to MyChart Technical Support. Please note that this information is required in order for us to research the issue for you. Not including the required info may delay our response to you.
